Liability waiver

Bounce n’ Boogie LLC Release and Waiver of Liability and Indemnity Agreement

In consideration of being allowed to participate in Bounce n’ Boogie LLC Activities the undersigned, on his/her behalf, and on behalf of the participant(s) identified below, acknowledges, appreciates and agrees to the following conditions:

I am/represent the parent/legal guardian of the participant(s) named below, or I have obtained permission from the parent/legal guardian of the participant(s) named below to execute this agreement on their behalf.

I AGREE that the participant(s) names below and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any Bounce n’ Boogie LLC Class or Activity, such as but not limited to instructional classes, birthday parties, open play times, ActiveME Camps, Parents Night Out, and etc.

I am AWARE that there are inherent risks associated with participation in Bounce n’ Boogie LLC activities and I, on behalf of myself and the participant(s) named below, knowingly and freely assume all such risk, both known and unknown, including those that may arise out of the negligence of other participants; and, I, for myself and participant(s) named below, and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless Bounce n’ Boogie LLC, its members and owners, any Bounce n’ Boogie LLC volunteer or paid instructor, and 141 Osuna, LLC owners, partners and operators from and against any and all claims, injuries, liabilities or damages arising out of or related to our participation in any and all Bounce n’ Boogie LLC activities.

I hereby consent to the use of any pictures of my child participating in a Bounce n’ Boogie LLC activity, or statement by my child concerning Bounce n’ Boogie LLC activities, for Bounce n’ Boogie LLC advertising and promotional purposes.

I have read and agree to all terms of this release.
