A correction is quickest to act on when it carries the link to the record you are writing about, a link to the document as the company publishes it, and one line on what is wrong. Please do not send a copy you have signed — a filled-in waiver carries your name, your signature, and sometimes a child’s medical details, none of which belong in a public index.
